1. Sponsor
This giveaway ("Giveaway") is sponsored by Revive Wellness, located at 3728 91 St. NW, Edmonton, AB, T6E 5M3
2. Giveaway period
The Giveaway begins at [12:00:00 a.m. MT on Month DD, YYYY] and ends at [11:59:59 p.m. MT on Month DD, YYYY] (the "Giveaway Period"). Sponsor's timezone is the official timekeeper.
3. Eligibility
The giveaway is open to legal residents of Alberta, Canada who have reached the age of majority (18 years) as of the start of the Giveaway Period. Employees, contractors, and agents of the Sponsor, its affiliates, and the immediate family members (parent, child, sibling, spouse/partner) and members of the same household of such individuals, are not eligible to enter or win.
Void where prohibited by law.
4. How to enter
There are two ways to enter — no purchase is necessary to enter or win.
Method A — Purchase entry
During the Giveaway Period, entrants who purchase an eligible core service at Revive Wellness will receive one (1) entry for every one dollar ($1.00 CAD) of the pre-tax purchase price of eligible services.
Method B — No purchase necessary (free entry)
Entrants who do not wish to make a purchase may receive one (1) free entry by choosing the following:
Mail-in written entry
Entrants may mail a hand-printed or typed response of no more than 500 words, in English, describing (a) their health and fitness goals, (b) what they want to achieve over the next one to two years, and (c) why a fitness tracking device would help them achieve those goals, along with their full name, complete mailing address, phone number, and email address, to [mailing address], postmarked within the Giveaway Period.
By submitting a written entry under this option, the entrant grants Sponsor a non-exclusive, royalty-free licence to reproduce and share excerpts of the submission (with the entrant's first name and city only, not full contact details) on Sponsor's website, social media accounts, and marketing materials, for the purpose of administering and promoting this Giveaway, unless the entrant indicates on their submission that they do not consent to this use — declining does not affect eligibility to enter or win.
Each entrant may submit only one (1) entry under Method B, valid for one (1) entry, and each valid free entry received under Method B is worth one (1) entry, identical in weight to a Method A entry. Sponsor will not accept screenshots, DMs, or private-account submissions in place of a public, tagged Instagram post, as these cannot be independently verified, and will not accept incomplete or illegible mail-in submissions.
Illegible, incomplete, fraudulent, or mechanically reproduced entries are void. Sponsor is not responsible for lost, late, misdirected, or incomplete entries.
5. Prize
One (1) prize is available to be won: one (1) smart fitness tracker, with an approximate retail value ("ARV") of $550 CAD. Actual retail value may vary at time of prize award; any difference between stated ARV and actual value will not be paid to the winner. The prize must be accepted as awarded, is not transferable, and no cash alternative will be given, except that Sponsor reserves the right, at its discretion, to substitute a prize of equal or greater value.
6. Odds of winning
Odds of winning depend on the total number of eligible entries received during the Giveaway Period.
7. Winner selection and notification
On or about [date], Sponsor (or its designated representative) will select one (1) potential winner by random draw from all eligible entries received during the Giveaway Period (Method A and Method B entries combined and weighted equally).
The potential winner will be notified by [phone and/or email] using the contact information provided at entry. The potential winner must respond within [5–10] business days of the first notification attempt, or the prize will be forfeited and an alternate potential winner may be selected by random draw from the remaining eligible entries, time permitting.
8. Skill-testing question
Before being declared a winner and receiving the prize, the potential winner must first correctly answer, unaided (without the use of a calculator or other assistance), a time-limited, mathematical skill-testing question administered by the Sponsor. Failure to correctly answer the skill-testing question within the time allowed will result in disqualification, and Sponsor may select an alternate potential winner from the remaining eligible entries.
9. Winner declaration and release
As a condition of receiving the prize, the potential winner may be required to sign and return a declaration and release confirming compliance with these rules, eligibility, and correct completion of the skill-testing question, and releasing the Sponsor, its affiliates, officers, directors, employees, and agents from any liability associated with the Giveaway or the prize.
By entering, entrants consent, where applicable law permits, to the use of their name, city of residence, and statements regarding the Giveaway for promotional purposes related to this Giveaway, without further compensation, unless prohibited by law. Entrants may decline consent to this use without affecting their eligibility to win.
10. General conditions
● Sponsor reserves the right to disqualify any entrant found to be tampering with the entry process, violating these rules, or acting in an unsportsmanlike or disruptive manner.
● Sponsor is not responsible for technical failures, lost or delayed entries, or errors of any kind, whether human, mechanical, or electronic.
● By entering, entrants agree to be bound by these rules and by the decisions of the Sponsor, which are final and binding in all matters relating to the Giveaway.
● This Giveaway is in no way sponsored, endorsed, or administered by, or associated with, Meta Platforms, Inc. or Instagram (see also Method B above).
11. Privacy
Personal information collected in connection with this Giveaway will be used solely for the purpose of administering the Giveaway (entry validation, winner notification, and prize fulfillment) and will not be used for any other marketing purpose without the entrant's separate, explicit consent. See [Sponsor's privacy policy link] for details on how personal information is collected, used, and protected. If entrants are asked to opt in to future marketing communications, that consent must be collected separately from Giveaway entry, consistent with Canada's Anti-Spam Legislation (CASL).
12. Governing law
This Giveaway is governed by the laws of the Province of Alberta and the federal laws of Canada applicable therein.
